Request for Services

Please select the form that best meets your needs. Community Health submissions are requests for nurse volunteers to perform health screenings and education activities in the community. Sponsorship requests are for SDBNA to support local organizations through financial contributions, event participation, or promotion. Nurse speaker requests are for addresses to an audience or media outlet. Member recruitment events are opportunities for SDBNA to engage with potential members.

Please keep in mind that SDBNA is a fully volunteer-run organization and we receive many community requests. We make every effort to honor community engagement requests made with 60 days notice or more. Requests may be denied if we have conflicting commitments, lack of volunteer availability, or at the discretion of the relevant committee or board of directors.
